GP & Advanced Nurse Practitioner Appointments
We operate a total triage system for GP, ANP and clinical pharmacist appointments. All requests for appointments will be triaged by a GP before an appointment is given. Please complete an electronic consult form below. Your form will be reviewed by a GP and you will receive a text with an appointment time or signposting information. Please include all relevant information as this will help us triage your appointment appropriately. We understand that a small amount of patients will be unable to complete this and if that is the case please telephone the surgery on 01377 272747.

Advanced Nurse Practitioner
Our Advanced Nurse Practitioners see similar patients to our GPs. These are highly trained nurses, who can prescribe.
Practice Nurses
The Practice Nurses are able to carry out injections, cervical screening, general health screening and much more! Appointments are available with Practice Nurses on week days, morning and afternoon at The Park Surgery and Monday afternoons and Thursday mornings at the Nafferton Surgery. All of these appointments are bookable in advance and we strongly recommend you book a couple of weeks in advance.
Physiotherapy
You can access NHS physiotherapy directly by calling 01377 208300 or by referring yourself online.
Mental Health Services
Stressed, anxious or feeling down?
We all sometimes feel stressed, anxious or down from time to time, but if you have been having these feelings for a few weeks or more you may benefit from our support.
You can access talking therapies and help with mental health issues by calling 01482 335451 or by referring yourself online to Emotional wellbeing service
Sexual Health Clinic
For contraception advise and STI Testing you can alsocontact conifer house. Please note we no longer fit or remove coils and implanons - these services ca n be accessed through conifer house.
Home Visits
The best place to see a doctor is nearly always at the surgery as we have the necessary equipment and facilities available for safe, effective examination and treatment. Additionally, home visits are time-consuming and doing unnecessary visits would reduce our ability to see greater numbers of patients at the surgery.
However for patients who are housebound (ie. completely unable to leave the house even with transport), this is possible.
If you do need a home visit, please call reception or complete an online form before 11.30. We have no capacity to undertake late visits and illnesses that develop in the course of an afternoon likely require an emergency service.
All requests for home visits will be triaged by a member of the practice team in order to assist the GPs and nurses in prioritising visits. Your GP will also decide if/how urgently a visit is needed.
We regret that we are unable to offer home visits for patients who are prevented from attending the surgery by lack of transport.
Hospital Referrals
Please be aware that we have no control over hospital appointment times, if you need to check on the status of your referral please contact the relevant hospital via their switchboard asking for the secretaries of the department to which you have been referred. If you are unhappy with a hospital appointment or the waiting times please contact PALS.

Teaching Practice
There is usually several GP Registrars (trainee GPs) with the practice. They are all fully qualified doctors gaining experience in their chosen speciality of family medicine.
During their period with us, these doctors are asked to video some of the consultations. In addition, we work with the local medical school and host some student doctors.
We are grateful for your help with our teaching.
