New Patient Registration
We are a friendly practice and welcome you to join us. You can do this easily by bringing in your NHS medical card to reception. If you do not have one, we have a simple form for you to complete.
We do require you to provide some form of ID so we can verify the information you are giving us. We would prefer this ID to include a photograph however if you do not have this kind of ID, we will accept alternatives. Please ask at reception for further details.
If you register online, we ask you to bring ID to the practice on your first visit so that we can complete your registration.
As a patient you register with the practice, not one particular doctor. However you are entitled to express a preference as to which doctor you see and we will try to ensure that you see this doctor whenever possible.
When you register at the practice, we like our patients to complete a short questionnaire. This gives us some basic data to enter onto your medical records. If you need to make an appointment with a doctor or nurse at this time, you are able to do so.
If you live more than a mile away from a chemist, we can supply any medication prescribed for you.
Medical treatment is available from the date of registration. Please contact reception for further information.
